We’ve spent years teaching machines to think. Now we’re teaching them to collaborate — and they’re making the same mistakes we do.

A new research paper introduces MAST — the Multi-Agent System Taxonomy — a structured way to study how teams of AIs fail when they try to work together.

While each AI is strong individually, their teamwork breaks down in surprisingly human ways. Across 200+ AI conversations, the researchers uncovered 14 recurring failure types, grouped into three big families:

1️⃣ Specification problems — The system misunderstood the task or forgot what it’s supposed to do.

Examples:

  • Ignoring part of the task
  • Repeating steps unnecessarily
  • Forgetting context
  • Not knowing when the job is done

2️⃣ Inter-agent misalignment — the AIs disagreed or talked past each other.

Examples:

  • Ignoring feedback
  • Failing to clarify when confused
  • Resetting the conversation for no reason
  • Getting derailed

It’s like a group project where everyone has a different understanding of the plan.

3️⃣ Task Verification Problems

These happen at the end — when agents don’t check if the result is correct.

Examples:

  • Stopping too early
  • Skipping double-checks
  • Claiming something’s right when it isn’t

It’s like submitting homework without proofreading.

This sounds like a dysfunctional workplace meeting !

The most striking finding?

Many failures weren’t about intelligence — they were about structure.

When AI teams were reorganized — with clear roles, defined authority, and explicit verification steps — performance improved significantly.

It’s the same insight great companies already know:

Smart people (or machines) still fail if you give them fuzzy goals and unclear roles.

All three categories matter equally.

  • No single “root cause” — failures were balanced across the board.
  • Design is greater than (>) Prompt Tweaks. Changing who talks to whom and who can finish the task mattered more than changing wording.

The best systems had a clear Verifier role that checked others’ work before wrapping up.

The researchers trained another AI — a “judge model” — to identify these failures automatically. That means future AI teams could self-monitor their own mistakes in real time.

Imagine a code-writing AI that says:

“I think we may have misunderstood the spec — should we clarify before proceeding?”

The Broader Implication

The next leap in AI performance will come from better teamwork, not bigger brains. Because AI “teams” fail in human-like ways — confusion, miscommunication, rushing, poor memory.

The future may belong not to the biggest models, but to the best collaborators.
